A note to returning providers: If you have already entered a case into the registry, please do NOT start the registry entry from here. Instead, please use the individual survey queue link that was emailed to you after you completed that case. The email would have come from RheumCOVIDRegistry@ucsf.edu. If you cannot locate that email, please send a note to this email and they can re-send the survey queue link. It is fine to have another staff member enter cases on your behalf using this link.
