English text and purpose of the Pediatric Survey materials

The materials invite adults who care for a child younger than 18 with a pediatric rheumatic disease to share the child’s experiences with COVID-19 infection and vaccines. The survey states that participation is voluntary, takes about 15 minutes, has no cost or payment, and does not collect names, email addresses, IP addresses, or other easily identifying information. Additional flyers provide the same invitation in multiple languages; selecting a flyer opens the full-size version.
